Job Description:
A company is looking for a Licensed Practical Nurse (LPN) or
Licensed Vocational Nurse (LVN) to work remotely in a clinical
role.
Key Responsibilities
Manage patient census and close gaps in clinical and non-clinical
patient care
Provide patient education and coordinate community resources for
chronic condition management
Assist with medication management, referrals, and appointment
scheduling
Required Qualifications
Active Multi-State/Compact License (LPN) or (LVN)
At least 3 years of direct patient-facing work experience
Technical aptitude with Microsoft Office Suite
Experience with chronic care management
Ability to work in a HIPAA-compliant home office environment
