Job Description:
A company is looking for a Registered Nurse - RN - Case
Management - Diabetes Educator - Remote.
Key Responsibilities
Assess, plan, implement, and coordinate case management activities
for members to enhance their overall wellness
Utilize clinical tools to evaluate members' needs and develop
strategies to reduce risk factors
Engage with members using motivational interviewing techniques to
assess health status and needs
Required Qualifications
Registered Nurse with an unrestricted nursing license and
multi-state/compact privileges
3+ years of clinical/nursing practice experience
3+ years of experience in diabetes management
1+ year of experience in care coordination or discharge
planning
Associate's degree in nursing required; BSN preferred
